The medical explanation in lay terms are: tobacco smoke and its thousand or more chemicals paralyzes the lungs' normal cleaning mechanism and the lungs are then forced to - provide a heavy mucus production to help compensate. The mucus can only be moved out of the lungs by coughing, hence a ''smoker's cough!’.
